Dina earned her Master of Science in Nursing (MSN) and Family Nurse Practitioner certification from Chamberlain University, equipping her with advanced clinical skills and extensive knowledge across a wide range of healthcare specialties. She is board-certified and passionate about providing high-quality care for individuals of all ages.

Throughout her career, Dina has worked with diverse patient populations, helping them navigate a variety of health concerns, from acute illnesses to ongoing medical conditions. Kytori Tolbert, MSN, APRN-FNP, is a compassionate and results-driven Family Nurse Practitioner with over 15 years of experience in acute care, emergency medicine, and healthcare leadership. Her extensive clinical background and dedication to patient-centered care have shaped her into a trusted healthcare provider for individuals and families across the lifespan. She is passionate about promoting health, wellness, and preventive care while delivering exceptional, evidence-based treatment in both urgent and primary care settings.

Kytori earned her Master of Science in Nursing (MSN) from Walden University and is currently pursuing her Doctor of Nursing Practice (DNP), expected in 2026. She is board-certified and brings a wealth of expertise in acute care, chronic disease management, women’s health, pediatrics, and urgent care. Her ability to think critically, make rapid decisions in high-pressure situations, and advocate fiercely for her patients has been honed through years of frontline experience.

As a Family Nurse Practitioner, Kytori provides comprehensive care for patients of all ages — from routine wellness visits to managing complex medical conditions. She values strong, lasting patient relationships and works collaboratively to create personalized care plans that emphasize prevention, education, and holistic wellness. Amber Layson is an advanced practice registered nurse and board certified family nurse practitioner.  With a strong foundation in critical care nursing and a deep commitment to patient-centered- care, Amber brings a wealth of clinical knowledge and compassionate service to urgent and primary care patients.

Prior to becoming a nurse practitioner, Amber worked for over seven years as a registered nurse in an intensive care unit, where she developed a strong skill set in acute care, crisis management, and interdisciplinary collaboration. In addition to providing direct patient care, she held leadership roles and helped establish safe and efficient units in the hospital, demonstrating both initiative and commitment to improving care delivery. 

Amber earned both her Bachelor of Science and Master of Science in Nursing from Chamberlain University.  She is certified as a family nurse practitioner from the AANP.  She is passionate about delivering evidenced based, holistic care to patients across the lifespan.  Known for her dedication, empathy and excellent communication skills, Amber works closely with patients and families to anticipate needs and foster positive health outcomes.

Amber’s clinical excellence and compassionate approach have earned her recognition among her peers, including a nomination for the prestigious Daisy Award. She continues to build on her commitment to high quality care through continuous learning and by staying engaged with the evolving landscape of family medicine.
